Procedure to submit claim

An employee has to submit a Declaration Form (Form GCL1) with the relevant documents as early as possible before going on childcare leave. This is to allow the employer to verify his/her eligibility for the scheme. A declaration form will be required for each application of childcare leave within the relevant period. The employer is required to keep the form and the relevant documents for 3 years for audit purpose.

With effect from 1 Feb 2010, employers may use their own form or IT system to capture the declaration from the employee on his/her eligibility for GPCL. Please note that an employer who prefers to use the declaration form GCL1 may continue to do so.

An employer who opts to use its own form or internal system (in place of Form GCL1) has to ensure that:
a) ALL the following information is provided by the employee:
|
(i) |
the employee’s name; |
|
(ii) |
the employee’s Singapore National Registration Identity Card (NRIC) number or Foreign Identification Number (FIN); |
|
(iii) |
whether the employee is married, widowed or divorced; |
|
(iv) |
the name of the employee’s spouse, if the employee is married; |
|
(v) |
the Singapore NRIC number or FIN of the employee’s spouse, if the employee is married; |
|
(vi) |
the name of the employee’s youngest child who is a Singapore citizen; |
|
(vii) |
the Singapore birth certificate number or Singapore citizenship certificate number of the child mentioned in point (vi); |
|
(viii) |
the date of birth of the child mentioned in point (vi); |
|
(ix) |
the date or dates on which the employee wishes to take childcare leave; AND |
b) The employee declares that he/she is eligible for the childcare leave taken on the date(s) as stated in (a)(ix)

Timeline for submission of claim for reimbursement
|

The employer/self-employed parent (father or mother) should submit the claim for reimbursement within 3 months after :
- the last day of childcare leave taken; or
- the end of the relevant leave period.

Documents and information to prepare before submission of claim
|

The reimbursement will be credited into the bank account of the employer/self-employed parent. Hence, a Direct Credit Authorization (DCA) form (original form complete with bank’s endorsement) will need to be submitted to the Central Provident Fund Board (CPFB) at least 1 week before the claim is submitted online.

Please note that the DCA Form need not be submitted if any of the following conditions is met:
- The company/self-employed parent has a standing GIRO account for CPF contributions; or
- The company/self-employed mother has previously claimed for reimbursement of Government-Paid Maternity Leave; or
The company/self-employed mother has previously claimed for reimbursement of Government-Paid Adoption Leave; or
The designated account to receive the reimbursement is an UOB or OCBC bank account

8.3 |
Details of childcare leave taken:
- Relevant period for taking childcare leave (any 12-month period mutually agreed by employer and employee to take the childcare leave; - Dates of childcare leave taken; - Salary details of employee during the childcare leave dates; - Reason for claiming less than 3 days of GPCL
|
- Declaration form submitted by employee; - Leave records; - Payslips for the month(s) that the childcare leave was taken.

For self-employed parents submitting claim for reimbursement -

If the employee took less than 6 days of childcare leave (due to reasons such as ceasing of employment, ceases to be eligible for the scheme etc), the employer may submit the claim as soon as possible. They will need to provide the reason for not claiming the full 6 days of leave at the point of application for reimbursement.

The processing time for childcare leave claims is 15 working days after successful submission. Incomplete submission or inaccurate information would result in a delay in processing.
