For enhanced Government-Paid Maternity Leave scheme (16 weeks)
1 An employee has to submit a Declaration Form (Form GML1) with the relevant documents at least one week before going on maternity leave. This is to allow the employer to verify her eligibility for the scheme before granting the leave. The employer is required to keep the form and the relevant documents for 3 years.

2 Both the employer and employee should come to a mutual agreement on whether the maternity leave should be taken in a continuous block of 16 weeks or in a flexible arrangement. Please note that for maternity leave taken in flexible arrangement, the first 8 weeks of leave must be taken as a block.
3 The employer/self-employed mother should submit the claim for reimbursement within 3 months after the end of the maternity leave period.
4 The reimbursement will be credited into the bank account of the employer/self-employed mother. Hence, a Direct Credit Authorization (DCA) form (original form complete with bank’s endorsement) will need to be submitted to the Central Provident Fund Board (CPFB) at least 1 week before the claim is submitted online.
5 Please note that the DCA Form need not be submitted if any of the following conditions is met:
· The company/self-employed mother has a standing GIRO account for CPF contributions; or
· The company/self-employed mother has previously claimed for reimbursement of:
- Government-Paid Adoption Leave; or
- Government-Paid Childcare Leave; or
- Government-Paid Maternity Leave; or
· The company/self-employed mother has previously claimed for reimbursement of Government-Paid Adoption Leave; or
· The designated account to receive the reimbursement is an UOB or OCBC bank account
